Who is it for: All our Asthma & COPD patients who need monitoring on a yearly basis
How do you make an appointment:
- Via a yearly recall system
- At GPs request to ring reception
What Happens at the of Clinic:
- Monitor and improve the care of your breathing problems
- Advice on inhaler technique and medication
- Smoking advice
- Spirometry
- Review disease status
The clinic aims to improve patient's quality of life through better management of their disease using the guidelines of the British Thoracic Society. Our aim is to get you actively involved in the management of your disease through a greater understanding of asthma to enable you to lead a healthier life.
Flu vaccinations
When: Yearly Oct - Dec
Led by: Practice Nurse/HCA
Who is it for: Patients over 65, Patients under 65 with specific chronic disease
How do you make an appointment: Following a letter of invite ring reception
What Happens at the of Clinic:
- Health status and allergy check
- Vaccination given
- Written advice given on side effects
CHD
When: Monday all day, alt Wednesday afternoon. (also during some Diabetic visits)
Led by: Practice Nurse
Who is it for: All patients presented who present secondary heart disease
How do you make an appointment: Invite by letter annually six month after your birthday
What Happens at the of Clinic:
- Annual review - including Health Check
- Healthy Life Style Promotion
- Smoking Cessastion Advice when applicable
Diabetes Clinic
When: Monday, Tuesday, Wednesday, Thursday mornings
Led by: Specialist Diabetic Nurse
Who is it for: New & existing patients who have been diagnosed with diabeties
How do you make an appointment:
- Invite via recall letter
- At reception for newly diagnosed patients
What Happens at the of Clinic:
- Dietary advice
- Advice on home monitoring of your blood sugar
- Examination to check blood pressure etc
- Blood and urine tests
- Review of your blood sugar control and your medication
The aim of the Diabetes Clinic is to increase the awareness and understanding of diabetes through education and minimise complications by regular monitoring of the condition.
Rheumatology
When: Alt Wednesday mornings
Led by: Practice Nurse
Who is it for: Patients with Rheumatology disease who require blood tests
How do you make an appointment:
- Given next appointment when attending clinc
- New patients invited by telephone
What Happens at the of Clinic:
- Monitoring blood
- Previously blood results given
- Medication advice
- IM injection if required
